Description: The URMC Equity and Anti-Racism Action Plan outlines a vision that includes the delivery of equitable health care to our community. The Health Equity and Anti-Racism Technology (HEART) Program team heard the call and endeavors to implement and operationalize the electronic health record tools needed to achieve this goal. Through improved collection of data in the areas of demographics and social determinants of health (SDoH), we will be able to better understand our patients, their needs, and our performance in their care. This presentation will highlight how collecting and displaying SDoH data will help us continue connecting our patients to needed resources both within and outside URMC's walls.
At the conclusion of this presentation participants should be able to:
1. Recall what is considered a full demographic data set at URMC
2. Recount which SDoH domains will be available for collection and those that will be prioritized at URMC
3. Anticipate increasing data availability (demographic and SDoH) over the next four years (2025)
